Cycling Knee Pain: Tips & Tricks

Knee pain is a common complaint among cyclists, and bike setup, riding position and training load can all play a role. If something doesn't feel right, there are a few practical areas worth checking first.

Saddle Height & Position

Saddle height is the first thing worth checking. A saddle set too low tends to show up as pain at the front of the knee, around the kneecap — your knee is staying more bent through the pedal stroke than it needs to. A saddle set too high can pull the other way, showing up as pain behind the knee or in the hip, as you rock side to side to reach the bottom of each stroke. A reasonable starting point most fitters use: at the bottom of the pedal stroke, your knee should have a slight, comfortable bend — not locked straight, not still noticeably bent.

Fore-aft saddle position matters too. If the saddle sits too far forward or back relative to the pedals, it changes how your knee tracks over your foot through the stroke, which can create strain over time even if the height itself is right.

Cleat Position

If you ride clipped in, cleat position is worth checking before anything else on the bike. The fore-aft position of the cleat, the rotational "float" (how much your heel can move side to side before disengaging), and the angle of the cleat all affect how your knee tracks with every pedal stroke. A cleat that's rotated even slightly off your natural foot angle can translate into knee strain, particularly on the outside of the knee, over hundreds of pedal strokes a ride.

Cadence & Gearing

Grinding a big gear at a low cadence puts more force through your knees with every stroke. Spinning a lighter gear at a higher cadence spreads that same effort over more, smaller pedal strokes — generally easier on the knees, especially on climbs or when you're fatigued late in a ride. If your knees tend to flare up on hilly rides specifically, cadence is worth paying attention to before assuming it's a fit issue.

In the video above, Erin breaks down the anatomy behind anterior, or patellofemoral, knee pain — the type most cyclists deal with. Your kneecap sits and tracks in a groove at the front of the knee, and how well it tracks depends on the muscles pulling on it staying balanced. Her point: tight quads and IT bands paired with weak glutes are the usual combination that pulls the kneecap slightly out of alignment as you pedal. Ride on that imbalance long enough and the ache at the front of the knee is the result — left unaddressed, she notes it can progress to irritation of the cartilage or meniscus underneath the kneecap.

Her recommended order of operations: fix the bike fit first using the checks above, then address the muscle imbalance directly — loosen what's tight and strengthen what's weak, rather than stretching alone or strengthening alone.

Bike Fit, More Broadly

Saddle height and cleats are the two most common starting points, but knee pain is often a symptom of something in your overall fit — frame size, crank length, or general riding position — rather than any single adjustment. If you've checked the basics above and pain persists, a proper bike fit with a qualified fitter is the most reliable next step. A fitter can assess your position as a whole, which is difficult to do accurately on your own.

Sudden Changes in Training Load

Not every cause is about the bike itself. Ramping up your riding volume or intensity quickly — more distance, more climbing, more consecutive days in the saddle than you're used to — is a common and often overlooked cause of knee pain, and it's unrelated to fit entirely. If your pain started around the same time as a jump in your training, that's worth considering alongside any fit checks.

Riding Position

A very stretched-out, aggressive position can change how much work your knees do relative to your hips, particularly over long rides. If you've recently changed your position — a new stem, different bars, a more aggressive setup — and knee pain followed, it's worth reviewing whether that change suits the kind of riding you're actually doing.

Stretching & Foam Rolling, Done Right

Erin's guidance here is specific: hold stretches for 30 seconds to a minute, done post-ride and 2-3 times a day while you're actively dealing with pain. Anything under about 15 seconds isn't doing much — a muscle needs roughly 20 seconds to actually start to relax, so short holds don't get you the benefit. The main targets are your quads, hip flexors, and glutes. Foam rolling helps too, but technique matters — long, smooth, progressive passes rather than rushing back and forth over the same spot.

A 3-Phase Plan to Prevent It Coming Back

Beyond the immediate fix, Erin lays out a three-phase strengthening and mobility routine aimed at keeping patellofemoral pain from coming back:

  • Phase 1 — Strengthen your glutes: glute bridges, crab walks, standing glute kickbacks, and bird dogs — 3 sets of 6–10 reps, adding a resistance band as it gets easier.
  • Phase 2 — Increase hip range of motion: pigeon pose, pretzel stretch, deep lunge, and side adductor lunge — held 30–45 seconds per side, 2-3 times.
  • Phase 3 — Release your quads: lying quad stretch, twisted kneeling lunge, reclined quad stretch, and kneeling lunge — held 30–45 seconds, 2-3 sets per side.
